Applying for long-term care insurance benefits

The application for benefits must be submitted using a long-term care insurance benefits application form, which can be obtained from:

  • the National Health Fund (CNS)

The application for benefits comprises 2 parts: a form to be filled in by the applicant AND a medical report to be filled in by their primary care physician: the application for benefits will be considered complete only when the National Health Fund has received both parts.

The medical report (R20) is free of charge for the applicant: the physician will be paid directly by the long-term care insurance provider.

The applicant will be sent a letter confirming the receipt of their application and specifying the date of entitlement to the benefits. If, after the evaluation conducted by the AEC, the applicant is recognised as a dependent person, the care and assistance provided through a care and assistance network (benefits in kind) will be due from the date of application.
