Request for basic assistive technology via Helpline

Under the long-term care insurance scheme, certain basic assistive devices may be requested using a standard assistive device application form.

A standard application form for standard assistive device may be submitted on behalf of any person who has previously made a claim under long-term care insurance, whether for assistive technology or for care and support.

This application may be submitted by the beneficiary themselves or by a care and support provider authorised for this purpose. The beneficiary’s signature is mandatory.

If a person has never before submitted an application under the long-term care insurance scheme, or when an assistive device is required urgently under the health insurance scheme – particularly in view of an imminent return home following hospitalisation – please refer to the information provided by the National Health Fund (CNS) on the following website: cns.public.lu
